What is Medical Abortion?

Medical abortion/ medication abortion /“Abortion pill” is the common name for using two different medicines to end a pregnancy: mifepristone and misoprostol.

Mifepristone is taken to block the hormone progesterone, this stops the pregnancy from growing. A second medication, misoprostol causes cramping & bleeding to empty the uterus. Medication abortion is appropriate for low risk people who are less than 9 weeks as estimated by the last menstrual period.

3. Taking the medication at home

You choose when and where to start. Most people experience no side effects from mifepristone, but plan to stay home when taking misoprostol, as bleeding typically begins within a few hours and may last up to 24 hours. Have absorbent pads, a heating pad, pain medication, bland foods, and comfortable clothes ready. Many people choose a weekend for this step.

4. Follow-up and support

Cramping and bleeding after misoprostol are expected. If bleeding hasn't started within 24 hours, contact your Tia Care Team, as you may need an additional dose of misoprostol included in your prescription. Your Care Team will continue checking in through chat for a month.

Symptoms and side effects of a medication abortion

While most people have no side effects from the first pill, some people experience side effects from the second pill, misoprostol, which can include:
Gastrointestinal
Symptoms may include nausea, diarrhea, and vomiting. We recommend stocking up on comfort foods. For nausea or vomiting, it’s best to stick to bland foods like crackers, bananas, and broth as well as stomach- settling & hydrating liquids like ginger ale, mint tea, sports drinks, and popsicles.
Vaginal bleeding
You can expect heavy bleeding and some blood clots. We recommend using pads to allow everything to pass, making it easier to monitor your bleeding. Make sure you have some super absorbent or overnight pads for the first few days as well as some regular and panty liner sizes for follow up.
Uterine cramping
We recommend having a heating pad and some over-the-counter pain medications like Tylenol or ibuprofen on hand.
Fatigue
Plan to wear comfortable clothing and be in a place where you can rest & recover after taking your medication. We recommend getting a good book or watching some binge-worthy TV.

Frequently asked questions about Medical Abortion

How far along can you be for a medication abortion?

Up to 9 weeks, estimated from your last menstrual period. If you're further along or need more information to be sure, your provider can recommend an ultrasound or refer you to a local provider.

Who is eligible for medication abortion at Tia?

You'll need to be 18 or older, a current Tia member located in California or New York, less than 9 weeks pregnant, without a medical history that would make an in-person or procedural abortion safer, and have a secure mailing address for the medications.

How does the abortion pill work?

It's two medications. Mifepristone blocks progesterone, which stops the pregnancy from growing. Misoprostol, taken second, causes cramping and bleeding that empties the uterus.

Is the abortion pill the same as Plan B?

No. Emergency contraception prevents pregnancy before it's established. Medication abortion ends an existing pregnancy. They're different medications for different situations.

What are the side effects?

Most people have no side effects from the first medication, mifepristone, and can continue their normal schedule. Side effects come from the second medication, misoprostol, and fall into four categories:

  • Vaginal bleeding. Expect heavy bleeding and some blood clots. Bleeding usually starts within a few hours of taking misoprostol and can last up to 24 hours. Use pads rather than tampons so everything can pass and you can monitor how much you're bleeding — stock super-absorbent or overnight pads for the first few days, plus regular pads and liners for the days that follow.
  • Uterine cramping. Have a heating pad and over-the-counter pain medication like Tylenol or ibuprofen on hand.
  • Gastrointestinal symptoms. Nausea, diarrhea, and vomiting are possible. Stick to bland foods like crackers, bananas, and broth, along with settling and hydrating liquids — ginger ale, mint tea, sports drinks, and popsicles.
  • Fatigue. Plan to wear comfortable clothes and be somewhere you can rest and recover after taking the medication.

If you haven't started bleeding 24 hours after taking misoprostol, contact your Tia Care Team. Often this means taking the additional misoprostol included in your prescription. Your Care Team follows up by chat for the first month after your visit, so reach out any time something doesn't feel right.

How do I manage the side effects?

For nausea, stick to bland foods and hydrating liquids like ginger ale, mint tea, or broth. For cramping, a heating pad and over-the-counter pain medication like Tylenol or ibuprofen. For fatigue, plan to rest.

Do I have to come into a clinic?

Your consultation can be done virtually, and the medications come by mail. If your provider needs further evaluation to confirm eligibility, they may recommend an ultrasound at a Tia clinic.

What if I'm not eligible?

Your provider will refer you to a preferred local abortion provider based on what you need. Tia also provides counseling across all options — parenting, adoption, and termination.

What our patients say about Tia

Jessica O., Tia Patient
"I cannot tell you how much I truly valued my experience here at this clinic. From the minute I walked in, I felt safe, comfortable and welcomed. The exam room had so many amenities to make you feel at home, I have a hard time calling it a "exam room" - they had amazing robes to wear, drinks/snacks just in case, and even a little sitting area in the room. My nurse was the most incredible woman and held my hand throughout the exam...everything was just perfect. I highly recommend Tia to every woman who wants to have a better doctor experience. This is unlike anything I have experienced and will be recommending every woman I know to Tia!"
Stephanie S., Tia Patient
"It's always easy and I've never felt so taken care of in my life! I actually don't dread making appointments and I don't have to put on armor when I go see my doctor. It's a game changer!"
Katherine E. Tia Patient
"I can’t recommend Tia enough. As a woman who doesn’t like going to the doctor (white coat syndrome) and has been discounted and not listened to by doctors in the past, this experience is everything a doctors appt should be. I am so happy I am now a Tia patient and would highly recommend Tia to any female friend or family member that could benefit from the amazing staff and care here 💕"
Whitney K., Tia Patient
"I’ve never experienced such personalized, sensitive, thorough, and holistic women’s healthcare in my life. So grateful for this clinic!"
