Our clinical outcomes are a reflection of our care philosophy and are a tangible way that we bring our care model to life during every single patient interaction.

Our clinical quality metrics focus on four core elements of preventative health: sexual health, cancer screening, chronic condition management, and mental health. These metrics go beyond numbers; they serve as practical tools to enhance health outcomes and help patients take charge of their futures.

Measuring for Change: Why Do Metrics Matter?

Tracking our quality metrics not only provides visibility into our overall performance, it also shines a light on gaps in care, pinpointing areas for improvement, and helping us set tangible goals for the future. Ensuring Tia patients receive exceptional care at every visit and during every interaction is our number one priority.

As an example of bringing this innovation and commitment to life is , towards the end of 2024, we identified an opportunity to ensure that more of our patients with diabetes had up to date A1C lab results in their records. To address this, we ran an outreach experiment via chat encouraging our patients living with Diabetes to to schedule follow-up appointments with their Tia providers in order to get updated labs, and receive additional support. 75% of patients that booked a follow-up appointment got updated labs as a result. This small yet impactful step ensured we were fully supporting patients in managing their diabetes.

Metrics also serve as valuable feedback for our clinicians and clinical leadership team. They guide us in refining clinical best practices, ensuring our care model evolves alongside patient needs. With our “experiment always” mindset, we’re quick to identify gaps and test new approaches to better engage patients in their preventative care journeys.

Our metrics empower us to lean into preventative care, which has a far-reaching impact on overall population health as well as long-term healthcare costs.

Preventive Health Clinical Quality Metrics

1. Cancer Screening: Detecting Early, Saving Lives

Cancer screenings are a cornerstone of preventative care, and for good reason. Early detection can significantly reduce mortality rates and improve outcomes. At Tia, we are committed to ensuring patients under our care receive routine screenings for cervical cancer and also receive the necessary follow-up for abnormal results.

Cervical cancer remains one of the leading causes of cancer-related deaths among people with a cervix in the U.S. That’s why ensuring timely access to screenings and follow-ups is a top priority for us.

2. Sexual Health: Prioritizing Overall Wellness

Sexual health is an integral part of overall well-being, and we prioritize it within our care model. Often stigmatized and forgotten - at Tia, we ensure routine screening for chlamydia, HIV and Hepatitis C and integrated into primary care.

Preventative measures like these enable early detection and treatment, reducing long term disease and curbing the spread of infections. By integrating sexual health services into our primary care model, we aim to work with the whole patient, addressing every aspect of their health and wellness.

3. Chronic Condition Management: Supporting Long-Term Health

Chronic conditions like diabetes and hypertension can feel overwhelming, but with the right support, they don’t have to be. Successful management of chronic diseases saves lives. At Tia, we work with each patient to develop a care plan that aligns with their goals and enables them to live their healthiest life.

We prioritize early detection and holistic management to prevent complications, incorporating non-pharmacalogical interventions whenever possible. For example, one of our success stories involves a patient with diabetes whose Tia provider recommended improving their sleep habits as part of their treatment plan. The result was a noticeable improvement in their A1C levels, without any additional costly medication.

We actively track metrics like glycemic control for patients with diabetes and blood pressure levels for those with hypertension. These insights not only help us tailor individual care plans but also inform broader care strategies to reduce the long-term burden of chronic conditions.

4. Mental Health: Embracing Holistic Care

Mental health is a core component of overall wellness, yet it’s often treated as secondary. At Tia, we’re working to change that. Mental Health is healthcare.

We measure key metrics like depression screenings and quality of life to ensure patients receive the support they need. For those who screen high for depression or low on quality of life, we track follow-up improvement over time to assess the impact of our interventions.

We monitor improvements over time in individuals who initially screen with high levels of depression or low quality of life scores to evaluate the effectiveness of our interventions.

We’ve invested deeply in ensuring our primary care team has expanded knowledge around how to manage mental health in primary care. One patient returned to Tia after a year away, struggling with severe anxiety and depression after a traumatic event. Through multiple visits with their primary care team, they built a holistic treatment plan that improved their well-being and re-engaged them in routine medical care. Their care team also coordinated a referral to a Tia Psychiatric Nurse Practitioner to ensure continued support. In messages to the care team, the patient thanked all of their providers for “actually listening”.

Our Nurse Practitioner-led Psychiatry service has also been a standout offering for our patients who need a higher level of mental health support. It remains one of our most highly utilized and highly rated service lines—reflecting the growing demand for accessible, empathetic, high quality mental health care.

These efforts ensure our approach to mental health is both comprehensive and patient-centered.
